Building Use Forms

Make sure to fill out both forms when requesting to use the building.

 

The church office will contact you regarding approval and any further communication that may be needed.

 

Building and Equipment Use Form

1. The use of the building and/or equipment must be approved by the Committee of Building & Maintenance and be scheduled by the Administrative Assistant. The Pastor will handle the approval and details of Funerals with the Session approval of Weddings. 2. There is to be no smoking, no consumption of alcoholic beverages or illegal drugs on the premises. Any food or beverages must be consumed in designated areas. 3. Those using the building and/or equipment will be liable for any damages that may occur during usage and the equipment must be returned where it was found. 4. There is not a charge for use of the building and/or equipment for our members or non- profit or community organizations. However, it is expected that payment is made for any additional expenses of maintenance or extra services required by Sexton. The building must be vacated by 9:00 pm unless otherwise stipulated. 5. The fees for non-member groups or for-profit organizations will be determined by the Committee of Building & Maintenance based on the need for rooms, equipment and staff assistance. In the event that this application is approved, but for any reason, the building is not available at the time agreed for the proposed use, the complete and total compensation to the applicant for the unavailability will be the return of fees paid, if any. 6. The attached indemnification agreement shall be signed by the Event Coordinator of every group or organization that uses the building or equipment. 7. You will receive a response from the church to your request.

Building and Equipment Indemnification Agreement

In consideration that the Second Presbyterian Church of Newark, Ohio, shall permit the undersigned to use or rent a part of the building or any equipment, we do hereby agree that we will indemnify and hold harmless the church, pastors, staff, trustees, officers and members against any and all claims, liabilities, damages and losses, including expenses incidental to the defense of the same, resulting from injury or death of any person or damage to said property during or as a result of the occupancy of said premises, or use of any equipment, and that neither we, nor anyone claiming through us, will hereafter bring, commence, prosecute or maintain, or permit to be brought, commenced, prosecuted or maintained, any suit or action in court, against Second Presbyterian Church, for or on account of any injuries or damages arising out of or during our use of said premises. Further, in the event that permission to use or rent a part of the building or any equipment is granted and applicant has paid fees required, but for any reason, the building or a part of the building or any equipment is not available at the time agreed for the proposed use, applicant agrees that the complete and total compensation to the applicant for the unavailability will be the return of fees paid if any.
